Proof of Residency and No Fee Identification Card

District residents who are homeless are eligible for a free DMV non-drivers identification card.  This form when certified by an approved social service provider, shall be used to certify the applicant's residency at the certifier's address.  All necessary documentation for proof of identity, birth and social security are still required from the applicant to receive a non-drivers identification card.
