The Application
for Disability Parking Placard or Tags* is available at any
DMV service center location
and on this website. You must be a DC resident to obtain disability
tags.
The application includes a portion that must be completed by your
physician if you are applying for a disability tag. The application
completed by your physician may be mailed to DC DMV, Medical Review
Services, PO Box 90120, Washington, DC 20090, faxed to (202) 673-9908
or brought to any DMV service
center location.
If your application is approved, you may be issued disability tags.
By law, DC residents may be issued one set of disability tags and
a disability parking placard/permit. You have the option to receive
disability tags for one or two years based on your vehicle registration.
Please note that if you request disability tags, you must present
your current driver's license or non-driver identification card,
current vehicle tags (if not an initial application), and a vehicle
registration in your name before receiving the new tags.
There is no extra charge for disability tags. However, there is
the normal, annual registration fee for the disability tag. Also,
if you already have pleasure tags and are obtaining disability tags,
there is a $10 tag transfer fee. You must surrender any existing
tags to obtain disability tags.
