Replace a Commercial Driver License

If your commercial Driver license has been damaged, lost or stolen, you can obtain a duplicate commercial driver license. The DMV offers duplicate commercial driver licenses online or at a DMV service location.

The Patriot Act, Public Law 108-458, National Intelligence Reform Act of 2004 does NOT allow for social security numbers to be displayed on driver licenses or identification cards.

Requirements

When you are obtaining a duplicate commercial driver license at a DMV service location, you must bring the following documents with you and be prepared to pay the applicable fees.

• Completed Drivers License/Voter Registration Application [PDF]
• Proof of Identity
• Proof of Social Security Number
• Proof of Current Residency (If Changed)
