Replace a Provisional Driver License

If your provisional driver license has been damaged, lost or stolen, you can obtain a duplicate provisional driver license. The DMV makes obtaining a duplicate provisional driver license convenient by offering the service online or at a DMV service location. For the online transaction, you must possess a DC REAL ID provisional driver license (star located in upper right corner) or Limited Purpose provisional driver license (which is marked not for federal purposes). Your unique provisional driver license number, which is contained on previous DMV receipts or driver records, is necessary to maintain the security of your identity.  If you visit a DMV service location to obtain a duplicate, be sure to review the requirements for in-person replacement provisional driver license as the document requirements have changed, and you will need to bring additional items with you.

Click on all three (3) links below to review a list of required documents you must bring from each category and be prepared to pay the applicable fees. The DMV will NOT be able to accept documents that are not listed for source documents.
