Replace a Provisional Driver License

If your provisional driver license has been damaged, lost or stolen, you can obtain a replacement provisional driver license. The DMV makes provisional driver license replacement convenient by offering replacement services at DMV service locations.

Requirements

If you are replacing a provisional driver license at a DMV service location, you must bring the following documents with you and be prepared to pay the applicable fees.

• Completed Drivers License/Voter Registration Application [PDF]
• Proof of Identity
• Proof of Social Security Number
• Proof of Current Residency (If Changed)
