To submit a Medical/Eye Report online, you must first download the DC DMV Medical/Eye Report Form and have it completed by a licensed medical practitioner.

• Once the Medical/Eye Report Form is completed by your medical practitioner, scan and save it.
• Next, upload and submit the Medical/Eye Report Form online as an attachment via the DC DMV Medical/Eye Report Online Submission Form.
• The Medical/Eye Report Online Submission Form should be completely filled out and certified.
• Once received, DC DMV will review the Medical/Eye Report Form.  If approved, your DC DMV driving record will be updated accordingly. You will be notified of the status of your online submission via the email address you provided on the Medical Eye Report Online Submission Form.

Your Medical/Eye Report will be processed within 7–10 business days from receipt. Please allow up to 15 business days to receive your product before contacting DC DMV.
