Dealers may request duplicate titles on behalf of customers by providing the following documentation and paying the appropriate fee:

• Copy of customer’s driver license or non-driver identification card
• Vehicle Power of Attorney Form, found at the link below:
• Vehicle Power of Attorney

If an executed vehicle title is lost or stolen, you must request a copy of the title from the previous owner.
