*The DMV Director has the authority to require additional restrictions based on safety requirements.
A restriction can only be removed if you provide an updated Medical/Eye Report with the appropriate findings by your physician, with the exception of the Corrective Lenses Restriction which has separate requirements (for Corrective Lenses Restriction removal, visit dmv.dc.gov/service/screening-vision). The same report form can be used as either a Medical Report or an Eye Report, and is available at the link below:
