Reinstatement Application

Updated: March 2023

Drivers whose license or privilege to operate a motor vehicle was revoked as the result of an alcohol or drug related offense must submit a written application to DC DMV for reinstatement of your District of Columbia driver’s license or privilege to operate a motor vehicle in the District of Columbia. You must include proof of completion of a state certified alcohol/drug counseling program.

Note: MADD Victim Impact and driver improvement courses are not acceptable documentation for the state-certified alcohol-counseling program requirement.

To apply for reinstatement:

  • Complete the reinstatement application and email to [email protected]
  • Mail your completed application and supporting documentation to:

DC DMV Adjudication Services
PO Box 90120
Washington, DC 20090

  • During business hours, bring your completed application and supporting documentation to:

955 L’Enfant Plaza, SW
Promenade Level – Suite P100
Washington, DC 20024

A hearing examiner will review the information presented and make a decision based on applicable District of Columbia traffic laws and regulations. You will be notified in writing of the examiner’s decision.

If you are approved for reinstatement, you must pay all outstanding tickets and fees and a $98 reinstatement fee. Drivers licensed in the District of Columbia must provide proof of SR-22 insurance if the date of your conviction was within the last three years.
